call the insurance company and find out exactly what more information they
want.

My insurance company, BCBS TN, wanted proof of past medically supervised
and other attempts to lose weight. I also sent in letters of
recommendation from 4 medical professionals and a letter that I wrote.
Many time the insurance companies are stalling, in hopes that you will give
up the fight....do NOT let them win!! Best Wishes! Pam

The letter I received spelled out exactly what additional information they
needed from me.
First, read you policy and see what the requirements are. If it doesn't
specify, call the insurance company and ask them what the requirements are
and what additional information they need. If you didn't already provide
it to them, they could be looking for diet history (possibly dr.
supervised), medical records for 5 years or so in order to see record of
obesity, records documenting co-morbidities, etc.
Good Luck.

I have Cigna PPO and what they were looking for when they said "more
info" was they wanted a letter of medical necessity. I was able to get
one from my neurologist who diagnosed me with sleep apnea. Good luck!
