Does anyone have any info concerning getting approved if you have an exclusion?

I am helping a friend get approved for surgery. She has United Healthcare, with a written exclusion, but we found two people who had been approved despite this. One was ladybug71, but I can't get in touch with her. She said you need to know the name of a claims examiner, and send it straight to them. Can anyone help me??
