Appeal Letter?

okay i know i am probably jumping the gun but the insurance letter where is it i saw the insurance letter everyone that has crappy insurance has to write because they dont approve you the first time . i have dealt with this insurance for a little over a year and they dont pay real good so i thought i would prepare that second letter i dont remember what everyone calls it but i thought i would at least get a rough draft written up so when they say no i can appeal it thats it the appeal letter where can i find a good one i am sure i am going to need it    — blank first name B.
