Question:
Has anyone been approved by Rocky Mountain UFCW?

I'm not Rocky Mountain, but my husband is UFCW. They paid it all except my $5 co-pays and part of my ultrasound/ekg. Out of pocket was about $100. Letter from Doctor must state medical nescessity, and they'll send a pre-authorization for surgery. Your surgeon should help you with that paperwork.
