Question:
I was denied after request by PCP. I have to write a letter to appeal. Any ideas?

I have a letter to the Insurance company posted on my website at <a href="http://www.mywls.com/Insurance/Insurance_Letter/insurance_letter.html">http://www.myWLS.com</a> under "Insurance." I would suggest writing a letter like this, include your diet history through the years, family history of disease or obesity and back it up with your medical records of weight gain through the years from your PCP.
