Question:
I have Anthem Blue Cross Blue Sheild HMO/Plan code 332 and they have denied me twice.

There is no exclusion to my policy. And the customer service rep. said that I can't file another appeal. And she said she didn't know why I was denied. Amy, I also live in the Cincinnati area and have AnthemBC/BS. Your plan code sounds familiar however, I have a POS not an HMO. My insurance is through General Electric. I am using the same surgeon you are. I wrote my diet history and fortunately had lots of documentation concerning my Doctor supervised diets-which I was told was all that my ins. company needed. I was approved withing 72 hours on the first try. I would be happy to send you a copy of my diet history-it took me about a month to write. Feel free to e-mail me if you would like to correspond concerning this. I would love to help if I can. BTW my surgery isn't scheduled until 2/12/01.That's how busy Dr. Fallang is!!!
