Question:
Need example of letter from PCP

Hi, I plan to talk to my PCP on August 30th about getting the surgery again. The first time that I talked to her she was against me getting the surgery. She said that I was not heavy enough. So she asked that I follow a diet plan for 3 months and if I didn't lose the weight she would support me. I need an example of the letter she should write for me recommending that I have this surgery. All responses will be appreciated.    — Hope A. (posted on July 25, 2003)
