I have Anthem of Virginia. Most anthem plans are similar. I was told that
Anthem will cover RNY or VBG if the BMI is over 40 with no problems. Sure
enough, they day my surgeon's office called it in they approved it at the
same time. Good luck

I have Anthem BCBS (Virginia). They approved me and my coworker over the
telephone. It took less than 5 minutes for both of us to be approved.
THIS PLAN IS GREAT!

The only way to find out is to call Anthem BCBS NH and ask. Whatever they
tell you, tell them you want it in writing. My insurance company rep told
me over the phone that the surgery was not covered, but when I got the page
she was quoting from, I discovered she was wrong. I have Anthem BCBS of
Colorado and they had the following criteria. MO for at least 5 years, 3
years of physician supervised weight loss attempts and significant
co-morbs. Fortunately, I met those criteria and had no trouble getting
approved, but all insurance companies can have different policies from
state to state and even from one employer to another in the same state.

I have anthem BCBS of Virginia. My information was sent to them on April
11th. I called them every week to check the status and they always said
they had no information and nothing in the system. They said this every
week for 6 weeks and then I was approved. I received an approval letter in
the mail but was never told anything when I called. I think the best thing
to do is to have your surgeon handle everything.
