Question:
can anyone tell me what I will have to do to be approved with this insurance co?

You did not say what insurance company you have. I just got the approval from my insurance company today. Have your PCP write an medical necessity listing all your co-mobilities, and all your failed atemps at weight loss and for how long, and call your insurance company and ask lots of questions, what they need, are you covered in your plan for WLS. Who or what surgeons are in your network, and just read peoples profiles and read the great information on this website. Hope this helps.

Hi there. There is resource info found under Local Help on your type insurance. I would also go to that link under Baratric docs and read what type insurances he's covered in past... then I would call the insurance and just ask what they need before it could be covered. Make sure to do research on which type surgery your interested in and mention that as well. Some insurances cover some but not all types of by-pass or lapband surgeries. Best wishes.

Hi, I have the same insurance company---and all I had to have was a diagnosis of morbid obesity---I had a BMi of 39.5 with a co morbidity of hypertension - so I qualified. HOWEVER, every company has their own contact with the insurance company--you need to just call the number on your card directly and specifically ask them what it takes to qualify. They also have a website: http://www.horizon-bcbsnj.com/ you can email them and get specific information pertaining to your individual plan. Hope this helps---and if they do include it--it only took 1 DAY for them to approve me--best of luck!
