Question:
Help - I need a sample letter from PCP to surgeon or insurance company

When I went for my consultation, I did not include a letter from my PCP. Since Aetna has changed their criteria (in the middle of my trying to get approved) I'm sure they are going to want this letter.<P>My PCP is wonderful and told me he would do anything he could to help me get approved. I even told him I would type the letter for him - how much better could it get than that? (He's having RNY in June - so he REALLY knows what it's like).<P>So, if someone has a sample I could use - it would appreciate it very much.-Margaret    — Margaret G. (posted on May 14, 2002)

Hi. You can click on the Your Journey link on the site to get to the footsteps page. From there you can click on the "getting authorization" footstep. That section has a sample PCP letter. Hope that helps!
