Question:
What else will I need for my initial visit?

I go to see the surgeon on Tuesday 10/8 and I have my LOMN from my PCP, I've written a letter to Cigna, I have a copy of my records from my old PCP from 2000-2001 who prescribed Xenical, and I have my weigh-in sheet from Weight Watchers from 6/9/02-10/12/02. My PCP has faxed over to the surgeon all of my records, including my EKG, ECHO, and Bloodwork, and my PSYCH consult letter has been faxed. What else will Cigna need?    — D. N.
