Hi there, I'm from the Chicago area and I have United Healthcare and was approved for the surgery with no problems whatsoever within 1 week of my surgeon, Dr. Joseph Vitello of UIC Hospital submitting my paperwork. The only qualification was to have a BMI of 40 or greater and mine was 40.7 on my initial consult and no further information was required for me to get approval. I am also from the Chicago area and have UHC. I had no problems getting approved. Once Dr Printen's office submitted the information i was approved in 3 days. Good Luck to you.
