Question:
Has anyone received approval from Sloans Lake PPO for LAP RNY ?

My insurance is PHCS-Sloans Lake PPO and they have exclusion. I am going into my consult w/ BMI of 40-41, sleep apnea, high chol/trigl, stress incont, and back pain. Any advise on getting approval ?! (Thanks!)    — Elizabeth D. (posted on May 18, 2002)
