Question:
Does anyone have a sample of a referral/surgery recommendation PCP letter

My PCP is very much supportive of my decision to have WLS. I asked him if he would write a letter of support that I can give to my insurance co. and surgeon (I figure anything in writing will be helpful)he said he would like for me to draft the letter and he would put it on his letterhead and make any corrections or additions needed. I'm wondering if anyone knows where I may find a sample of what this letter should include (other than my diets attempts and comorbidities)? Should I enclude information on the surgery itself?
