Question:
Well today I recieved the grievence forms today from Care Choices now what do I do

When my doctor asked for a referral it was for a consultation They denied it as not covered. The form really doesn't ask for much except what the complaint is (Denial for consultation) nothing else. Do I submit my PCP's orginial letter. Other doctor letters that I have been collecting about my co-morbidities, prescription printouts on all my drugs. What do I need, can any one point me in the right direction, last time my doctor did this for me I didn't know that there was an appeal process and gave up now I am determined. Also can any one tell me there is only one Care Choices doctor in Mi. he is on the side of the state do I have to go to him or can I go to Ann Arbor which is 30 min. away. then I will have some support when and if I'm going through this whole thing instead of being over there by myself.    — Colleen K.
