Im was approved by Anthem in 3 weeks. I had only 3weeks of supervised

diatecian for gestational diabetes.

My insurer does not require any proof of supervised diets - yet at least.
It just depends on your insurer.

It totally depends on your insurance co...I have United Healthcare Choice
Plus, and did not even need a letter from my PCP. My surgeon wanted a
history of previous diets, as well as an involved medical history filled
out, but I did not need a pysch eval either. I was approved at my initial
consult with a BMI of 42 and very few documented co morbidities. Good luck
to you!

Hi Crystal, I was approved without Dr. Diets. WHat I told them was I did
all the other diets, like Atkins,Grapefruit,Weight Watchers etc. This
worked for me to be approved right away. One thing that I know helped was
my PCP, who happens to be my OB/GYN wrote the leter for me and told them
she knew how I have struggled with diets and how hard I worked to lose
weight. I hope this helps. God Bless.
