I agree we need to know who you are to send them to you. Also, are you trying to appeal an insurance company or an employer. Do you have an exclusion, what does it say. I've got a couple I have been sending out. Had some success with them. You may want to e-mail me with your address and I'll send them to you.

I just put my appeal letter in my <a href="http://www.obesityhelp.com/morbidobesity/profile.phtml?N=Curl973014034">profile</a>. I was originally denied because of no evidence of life-threatening co-morbidities. Go figure.<BR>
