Denied- Need appeal ideas

I was denied, and wish to appeal. The denial letter states that I can make and appeal, and the doctor can make an appeal on my behalf. I have read a lot of appeal letters that are for doctors to send, but I am looking for more ideas to put in my own personal appeal letter. I can't seem to find many personal appeal letters from the individual to the insurance company.
