Hi Barbara. There is one that you can print out here at obesityhelp.com.
I think Denise gave you the link for it. And, yes, it does help and I've
had many restaurants honor it. Most kids meals are not wonderful when it
comes to the kinds of foods that we can eat post-op, but many restaurants
will also honor the card by preparing and charging for "half"
servings too. Bob Evans has a great little turkey dinner for kids that's
really quite tasty as well as nicely priced ($1.99). And I usually get the
salad buffet at Eat-N-Park for the children's price and believe me it's
more than enough! I appreciate the various restaurant's that I've
frequented honoring the card and it's come in very handy many times. It's
worth the effort to find one and present it to your waitress when you go
out to eat. I try to personally thank the management whenever possible for
helping me and allowing me to enjoy their establishment by honoring the

Out of frustration of waiting for my Doctor to offer the cards I bought a
laminating machine and a package of business card stock and printed my own.
As it turns out it is very nice and even looks nicer than the one a friend
of mine has that her Doctor gave her. Anyway, it was expensive and I
still have a whole pack of cardstock and laminating pouches left over. If
you are interested I would be glad to make you a card for just $1 to help
cover cost of the cards, postage and laminating pouch. Believe me this is
not a profit making venture. All I would need is your name, and your
