I have BCBS of Michigan.
I am have been told by two surgical weight loss center, that I have to produce documentation covering at least 12 consecutive months of formal weight loss attempts, through doctors giving me a diet and writing progress notes related to the diet or proof with a diet plan such a jenny craig Do anyone experience the same with BCBS — kimberly B. I have blue care network and had to do the same thing. First with the
insurance and then with my doctor. After showing them all this -had no
problems. My doctor was a really big help with the insurance co, and wrote
them and also told them that I would be healthy with less medical problems.

Yes, I also have bc/bs of MI and that is what I was told also. But since I
had all that documentation, I had no trouble. Weight Watchers, going to
your doctor for diet pills or a diet, anything on that line would work.

I don't have BCBS but my insurance company Principal required the same
information. I believe it is a pretty typical request, they just want to
make sure you have tried other weight loss methods first and that those
methods failed you.
