Question:
Can anyone help with an example appeal letter??? Denied after 1st submission.

I am currently trying to draft my first appeal letter (Aetna US Healthcare/HMO). I was denied 3 weeks after first letter was submitted. Their reason: Need 5 years medical history. There were 4 years submitted the first time when I was told they only required 2 years!! I know it's just a stall tactic but I'm desperate here!! I only want to have to appeal this ONCE. I have had my face shoved into a brick wall over and over and over again for numerous reasons over the last 8 months since I decided to talk with my PCP about WLS. I'm getting frustrated and I really need help!! If you have a copy, just a sample of an appeal letter that you or your doctor(s) may have written, if you could send that to me I would be SO GRATEFUL!!
