Distal means a lot of intestine has been bypassed and proximal means a
relatively small amount of intestine has been bypassed.
Distal may lead to better weight loss, but it has a higher risk of
nutritional problems. You need to be very careful to take in enough protein
and take your supplements religiously with either kind of bypass, but it's
especially important with a distal.

The way it's been described to me is that you're considered prosimal if the
surgeon measured the bypass from the stomach end and distal if from the
"other" end. Correct me if I'm wrong. It continues to be a bit of
a mystery.
