In answer to your question, yes, I wrote my letter of medical necessity and
took it to my doctor and he signed and dated it. I followed he sample that
the surgeon had given and just changed names, symptoms and issues and my
doctor signed it. Hope this helps.

The women at my Surgeon's office were not very helpful either. My Ins.
denied me the first time and wanted me on a 6mnth regulated diet. So I
recorded all I ate for 6mnths and my monthly weight-in. My Dr. office
copied all info taken at the visits and then I put it all together in a
notebook along with a few photos of myself. I even wrote them a letter
explaining why I wanted the surgery, ect. Doesn't hurt to stroke their ego
also.... Hope that helps, Good Luck! Just had my surgery Monday.

Deborah,
I would love to have a copy of the letter. My insurance has an exclusion
for WLS and thought going through insurance was hopeless. Any help is
greatly appreciated!
