I wouls suggest that if your doctors office does not have this information, you can post this Q. to people that are closer to you. Go to the main OH page @ the top go onto forums, click the 1st link (main board) now @ the top of that page you will see a box that says go local, find your state and post the question there. You may have more luck if you get people closer to you to respond ; ) ~Tiffany in CA
