My doctor and insurance want six months diet history, it doesn't have to be supervised just my own records but I haven't kept any, just generally trying to eat less go for walks make better food choices! Any suggestions for what to put or list? — HAILEY (posted on January 19, 2006)

Hi, my insurance required the same thing. All they are looking for is that
you are not looking into this lightly. I put on mine weight watchers, slim
fast, low carb, walking, low calories and so on. They are just looking for
your attempts prior to the surgery. I hope that helps.

I agree- although I had to show 2 years. But I did the same. I was able to
list all my diets, time frames, amount lost and regained etc. I put even
book based diets like Dr Phil Shape Up, Weigh Down- a religious based
program, Weight Watchers, Clinic 30 Program and so so many more. I got
approved first try and in less than 2 weeks.

Nice list of various diets you may have tried but can't remember. I also
listed diet supplements I have taken over the years. (i.e. Dexatrim, Phen
Phen etc.)

I sure know what you are feeling, This was the hardest part of doing the
paperwork. I finally just did as close as I could. I also use the web-site
and was thrilled when I found others had place their diet history in there
profile, it was a great help. Good luck.
I am still not approved but doing the 1 year supervised diet.
