does anyone have united healthcare choice plus hmo in rhode island?

i'm hoping maybe someone can help me with a strong appeal letter. i was denied gastric bypass because of an exclusion in our policy. i'm awaiting letters of medical necessity from my pcp as well as my weight loss surgeon. any help you can give me will be greatly appreciated. thank you!    — Kerry R.
