SURGICAL AND NON-SURGICAL TREATMENT OF OBESITY, EXCEPT THAT NON-SURGICAL TREATMENT OF MORBID OBESITY IS A COVERED HEALTH SERVICE WHEN PROVIDED BY OR UNDER THE DIRECTION OF A NETWORK PHYSICIAN. — TERRI B. (posted on December 23, 2003)

If this is exactly how it is printed, this does not say they won't cover
surgery for MORBID obesity, only excludes surgery for obesity! And they
will cover treatment of morbid obesity under the direction of a network
physician. You could very well point this wording out in an appeal letter
and be approved for surgery if you are morbidly obese.
