I have Humana Commonwealth Maximum Choice which covers Ky teachers and other state workers and they covered my rny and had the same guidelines for covering lap-band. It really depends on the contract your employer has with the insurer. The hospital that I work at has a contract with Anthem Blue-Cross which specifically excludes any bariatric surgery so it can really depend on the employer more than the insurer.
