Question:
My insurance wants a 5 year weight loose history?
They want it signed by a doctor and I dont have one, what do I do, please help me get this started or some directions to get what I need for the Insurance company I have Untied Helath Care Choice Plus thank you ,Stacey — stacey (posted on January 5, 2006)

My physician had me document everything that I had tried along the dates,
the amount of weight lost, and amount regained. He then submitted it with
my paperwork. Fortunately, I was able to give an account going back to 1997
when I first tried metabolife. Have you asked your physician for
suggestions? Usually they just want documentation from your physician that
he is aware of this being a last resort for you rather than your first
attempt at weight loss. Seeing a nutritionist helps as well.

Stacey,I too needed a weight loss history all I did was make a list of all
my diet attempts losses and gains and when . I did as best as I could I
know it wasn't exactly right but it did the trick. If you make that list
wouldn't your Dr. be nice enough to sign it for you ? If you have any
history with your Dr. he should know how hard you have tried to lose
weight . Good Luck.

I was fortunate enough to have been keeping a health/diete history for 2
years, to document why (I didn't know?) I didnt' feel well... Anyhow...
along w/the food doc, I had daily "don't feel well" inclusions,
etc, and it really did help me. I got accepted after 3 days.
