Question:
Does anuone have a copy of a drs letter of support for Lap Band surgery?
My PCP wrote a perscription stating that he is in support of surgery. That is the only thing he wrote. I don't think that will be enough for the insurance company. Any information is greatly appreciated. — dodgedt (posted on June 9, 2009)

I didn't save a copy of the letter of recommendation from my Primary Care
Physician, but I can tell you it was only about 3 sentences. She stated
that I have tried numerous diet attempts and with my medical history, she
thought I would be a stellar candidate for Lap-Band.
My letter was very simple and straightforward, you may be fine. I have
Harvard Pilgrim for insurance and I pretty much got approval within a week
of submission by my surgeon's office.
I had my surgery on May 28th. Good luck to you!

Here is a copy of the letter my PCP wrote for me to use when appealing
surgery denial. BTW I got an OK within a week.
To Whom it May Concern;
Ms. Mary Dettmann is a 66-year-old patient of mine, who has had
long-standing history of clinically severe obesity that has been resistant
to nonsurgical management. She has been overweight since high school. Her
current BMI is 54. She has been thoroughly evaluated by a multidisciplinary
team at the Legacy good Samaritan obesity institute and found to be a good
candidate for bariatric surgery. Patient has been evaluated at the Legacy
obesity institute that has included a physical therapist, dietitian,
psychologist. The process has determined that Ms. Dettmannis an appropriate
candidate for bariatric surgery. She has also been evaluated by Dr. Emma
Patterson as an appropriate surgical candidate.
lam therefore requesting on her behalf, that her insurance cover the lap
band procedure for her. Thank you for your consideration in this matter.
