Question:
I NEED TO SEND MY HUSBAND'S EMPLOYER EXPLAINING WHY SHOULD THEY COVER WLS!!!

I AM LOOKING FOR SAMPLE LETTERS TO SEND TO A SELF-FUNDED EMPLOYER. HE REP AT THE COMPANY EXPLAINED THAT IT IS A WRITTEN EXCLUSION, HOWEVER, IF YOU PROVIDE ENOUGH PROOF THE EMPLOYER WOULD COVER IF IT IS MEDICALLY NECESSARY. MY ONLY PROBLEM IS THAT I DO NOT HAVE A PCP. BECAUSE I CAN NOT AFFORD TO GO TO A DR ON A REGULAR BASIS. HOWEVER, I DO HAVE HIGH BLOOD PRESSURE AND TAKE MEDS FOR THAT. I KNOW I HAVE SOME SORT OF SLEEP DISORDER BUT HAVE NEVER BEEN TESTED. NOT TO MENTION, I AM VERY DEPRESSED. I AM 5 FEET 5 INCHES AND WEIGH 310 POUNDS. PLEASE HELP ME. THANKS    — PATRICE T. I was in the same boat as you. I wrote such a letter and mailed it to my husband's employer and never heard a word from them. Your more then welcome to a copy if you wish. Just e-mail me and let me know where you want it e-mailed and if you have MS Word.
