Wow, I'm surprised a PPO is such a problem! Dr Marchesini in Brazil charges ~$8000; that includes hospital and private nursing post-op. He is very well loved by his patients. Learn about DS at duodenalswitch.com and you can join the Dr M support group at http://groups.yahoo.com/group/DrMarchesini-DuodenalSwitch/. There is also Dr. Ungson in Mexico, about the same price; sorry I don't know much about him but you could find out at the Yahoo group. I hope you will look into the DS before deciding on what type of surgery to have. It is wonderful!!

Or if you're looking for the RNY instead, check out Dr. Aguirre. He's in Mexico, offer open RNY and is about $7000 I believe if you can get in before the end of the year, after that it goes up a bit.
