Question:
Ohip Application

Does anyone know what paperwork is required to apply to OHIP for out of country surgery? Any help would be appreciated. Thank you    — LM17 (posted on June 13, 2007)

Hi there, You need to go to the office where you get your health card renewed, go to reception and ask for a form for out of country surgery, take that to your family doctor and then your family doctor will fax that to ohip and you wait to hear from OHIP. Unfortunately it could take 4-6 weeks to hear anything from OHIP, so sit tight !!! Good Luck .....
