Question:
Can someone please
E-mail me or mail me a copy of their letter their pcp wrote for them that my pcp can go by. I have to have one stating medical necessity. I would appreciate it deeply. I have to submit it to my insurance and they have an exclusion unless I can provide medical necessity. I have all my records, but I have to have a letter from my pcp for my husbands employer, to make an exception for me....Please help. Thank You,,,,,,,,B — B D. (posted on July 17, 2003)

The AMOS archives have quite a few letters which may fit the bill. You may
want to check there. Also, you can go here

doctor modify and use this letter.

I just sent you a copy of my PCP's letter. Hope it helps!
My PCP and I wrote it together. First she asked me to write it and then she
tweaked it later.... I actually put this letter in my Memory Album because
it's the letter that got me approval and saved my life. Good Luck To You!!

Hi. I used the letter that Maggie suggested. I copied and pasted it to my
notepad then changed it to describe my conditions. I took it to my PCP to
give him an idea of the kind of letter I needed and he just signed it. Good
