HAVE TO HAVE PCP WRITE LETTER TO INS TO APPROVE REFERRAL??????

I had Bc/Bs Highmark Keystone and my PCP sent a letter of referal to the surgeon . Keystone has so far paid for everything(I'm Post-op).
