You would need to contact your insurance company and have them explain what it means. That way you know what you are told is correct.

Also go to the Medicare website and call Medicare as to this statement. When I got my LapBanding, CarePlus insurance tried to tell me it wasn't covered. But I researched at Medicare and CarePlus's sister insurance Humana. Proving it is covered by both fronts.
