I have the State of IL. medicaid card

Depending on your insurance is the deceiding factor and must be continious and some say can be with in the last 2 years so get the information from you insurance. IF all the requirements from doctor n insurance has been met. including some need a drs.visit for 6 months also with out missing a month inbetween. Looking at this maybe 6 to 7 months. If you have do other requirements do them and pulmanary n Cardo must be done with 6 weeks of surgury and some testing within 1 to 2 weeks of surgery date..Hope this helps. Your doctor's staff will help you with codes for certain things to call and ask insurance. Hope this helps.
