Question:
How do i get documentation for surgery, when I never actually join weight watchers?

Hello! I worked through my surgeons office. They had a sample letter that
I provided to my primary care doctor to follow. I had moved within that
last 2 years so my new doctor had no proff of my attending any other weight
loss plans. But the fact that I was obese, met the BMI standard and had
sleep apenea were more important. If your surgeons office doesn't offer
assistance, make an appointment and talk with your primary doctor. Most of
them have been through these roads before.

I had to have proof for 5 years and was able to use pics for 3 of them. One
had the date on the photo, the others I was able to pretty much pinpoint in
an attached description.
