Does anyone have Advantage Health/HMO

Does anyone have Advantage Health Chancellor/HMo or just Advantage Health HMO and been denied 2 times? Has anyone appealed in person? I have many comorbidities-Fibromyalgia, breast cancer radiation pain (4 years remission)-diabetes-GERD, hiatal hernia, plate and screws in ankle (which needs to be removed), arthritis, osteoarthritis, lymphedema from the breast cancer which I have had twice,HORRIBLE PAIN THAT HAS CAUSED ME TO HAVE TO BE ON STRONG PAIN MEDS, and a few other . I want off of all the meds My insurance reQuires 18 months of documented weight loss attempts which I don't have a whole 18 months or even close because the diets I alwats tried I didn't have to go to my doctor for. I am 5'5" and weigh 269 now bmi I think is 44 or so.I have letters of medical necessity from my doctors and I am still being denied. DOES ANYONE HAVE ANY SUGGESTIONS FOR THE APPEAL IN PERSON THAT I AM GOING TO MAKE AND HAS ANYONE EVER HEARD OF ANYONE MAKING AN APPEARANCE AT THEIR APPEAL> I need help in figuring out what to do or say.
