need help writting a letter to insurance?

I did a search online titled letter of intent. It will give you a sample
of a letter of intent. Just mark through the parts that are the words of
your doctor and go from there. It asks for medications and medical
problems. Hope this helps you.

I decided pictures were worth a 1000 words so I also included a sheet of
about 7 different pictures over the span of many years to prove that it was
a lifelong problem. It worked and I was approved quickly. good luck.

Tell them a day in your life of obesity. Tell them what its like getting
dressed. how hard to bend down to put on your socks, even tell them what
it's like using the bathroom. After you've told them all that tell them
how much money they will be saving after you no longer have to take your BP
medicine, or having to pay for CPAP supplies, how you won't need to go to
the doctor so often for all the aches and pains you have. It worked for
me. Good luck!
