RYN LAP insurance from J P Morgan Chase NC

Hi all! I'm trying to help my sister do some research on choosing an insurance company that covers and usually approves Gastric Bypass. I had the surgery 6 months ago and am thrilled with it (down 95 pounds!) And my little sister (she is 26) is interested in having it done too. She has just been hired on permenently with J P Morgan Chase in North Carolina, and has choices of insurance Co. to go with, they are the following 1 United Health Care PPO 2 United Health Care 3 BCBS (Empire blue?) 4 Health Net If anyone has had experience with any of these and works for Chase, or J P Morgan Chase, Please let me know of your experience with one of these carriers.
