Question:
Will my surgeon retrieve information from my pcp or is that my job?

Your surgeon's office will most likely request it from your pcp. However,
to be on the safe side, I would go and pick them up from my pcp MYSELF and
deliver them to your surgeon's office. These are very important to your
claim for insurance approval. You will save yourself time because office's
tend to drag things out when they have to request them and have them sent.
PLUS you will KNOW for sure that they have them in their hands. You really
have to get things done yourself if you want them done promptly and
correctly. Just be sure to call your pcp's office first and let them know
you need them so that they can have a day or so to get them copied. You
can't really just show up and expect them to drop everything and copy your
records on the spot. (I used to work for my pcp) Hope this helps!

What I have done previously for this type of situation is to write up my
request of EXACTLY what I need and then fax it to the doctors office, also
include in your letter what date/time that you will be by to pick the
copies.
Then follow up via telephone to make they received it also.

Your safest bet is to request your records from your PCP and deliver them
to your surgeon yourself. This minimizes the risk of one or the other
saying they didn't get the request or didn't get the records.
