Question:
DOES ANYONE HAVE ANY INFO ON PHCS IN AZ for plastics?

I am ready to have my plastic surgury for tummy tuck, thigh lift & breast lift but my company just changed ins companys & I don't have any info on the new one. I have PHCS in TN and they denied my tummy tuck, saying because it didnt hang down far enough and I didnt have any documented rashes cases on file. Im a nurse so I Know how to treat the rashes. My denial letter said they needed 3 months worth of documentation from the MD for treating the rahses. so good luck.
