HMO's are really difficult when it comes to WLS! Your best bet is to take the time to sit down and call them all and ask if they cover WLS and what kind. Ask also what qualifications they have. By this I mean, what does your BMI need to be, weight related health problems, etc. I just don't want to see you disappointed. It is difficult enough with PPO's, HMO's are not big on WLS.
