Question:
Insurance denied, need help with info on dr's notes

What kind of stuff should my pcp put on his notes about nutrient. He just signed yes that we talked about it. Any info on what he should put down on my 6 month diet notes would be appreciated.    — pugsley101 (posted on November 12, 2008)

My NUT have to give copoes of hr notes from each of my visit it included my weight and what we talked about on each visit. Did you go to a NUT or PCP? I would suggest you have your PCP sne copies of the notes he took for each visit and to make sure your weight is included from every visit. My PCP was not help at all she does not want me to have the Lapban because she feel it is not right and i can do it I just need to work harder. If I could have done it I would have done it I wish you luck keep pushing dont give up
