I believe they are looking for the clinical notes (the reports they write
up after your visits) from your appointments with the professionals you
have seen: the social worker, the nutritionist, the surgeon (if you have
seen him/her, etc.

Elena is correct, Clinicals are the notes in your chart from the doctor,
lab results etc..Also referred to as your medical history.

They aer also looking for notes from Primary Care doc, where you have
attempted to loose wt, you co-morbids, and their recommendations. Your WLS
will tell you everything they will need. If you do not have one yet, call
to set up apt. I my case, they sent me a packet to pre-qualify before even
seeing the doc.
