For having surgery? Yes most of the time, your BMI has to be 35 or higher
and from most of the people I have talked to it has been with health
problems as well...

It depends on the type of surgery that you are having, lapband or RNY or
other. The doctor and the insurance companies all have their own criteria.

Generally speaking Body Mass Index of 40 or greater without any
comorbidities (i.e. diabetes, hypertension, hypercholesterolemia, etc) and
great than 35 with comorbidities. BMI of greater than 40 roughly translates
into 100 lbs overweight.

My insurance requires a BMI of 40 or 35 with commorbidities.

You need to be at least 85-100 pounds overweight. You also need to have the
risk factors related to obesity such as diabetes, sleep apnea, reflux, and
joint problems. You also will have had to try to lose weight on your own.
