ANYONE HAVE ANY EXAMPLES OF DIET HISTORYS SENT TO INSURER THEY ARE WILLING TO SHARE?

There is a great "Diet History" on the Web Site www.myWLS.com. I just found out about this site on the DS mailing list. It looks great. There's also an example of an insurance letter. Christina (Chris) pre-op for DGB w/DS.
