Question:
What are the BCBS of IL (PPO) WLS requirements?
I am unable to locate anything on their website, and I haven't received a plan description from them yet since coverage just started at the beginning of the year. Any and all help is greatly appreciated. Thank you!! — Amanda W. (posted on January 13, 2006)

I suggest you call them. It is better to get the information right from
the source than to get it elsewhere!

I had BC/BS of Illinois with my last job. I was denied because although I
have a high BMI, I have no documented co-morbs nor do I have 12 months of
documented doctor supervised weight loss program history. They are very
difficult to deal with unless you have all those ducks in a row. If you
have three co-morbs or the 12 months, you probably won't have a problem, so
I would suggest that if you can cover those bases ahead of time you'll be
better off. It also is important to know if your individual company has
any restrictions. I wish you the best of luck in your journey.

Hello
Like the first person said, its best to call and ask, but, I do have BC/BS
IL PPO. They required of me for approval (which is still pending BTW):
12 mos. doctor supervised weight loss attempt
(includes seeing nutrionist every month, seeing personal trainer every 3
months, seeing the MD every month, and seeing a psychologist at the
beginning of the 12 mos and end.)
I am now waiting for approval. Supposedly my information was sent out
1/18/06. I followed their program to a t-as my surgeon's office has a
program specifically tailored to meet their requirements.
Read my profile for my info if you wish.
But I will recommend starting right away, and sticking with it. it will
give you valuable knowledge on yuor tool, how to eat properly, and support
of course. I lost nearly 30 pounds!
I am glad that I did the program, as well. Evan after all the waiting. I've
been fat all my life, so 12 months was nothing to me!
