Question:
anyone with greatlakes in mi. to share there criteria.

does anyone know if you have all the criteria plus more except not sure about proving the dieting with the dr. even though i have dieted all my life not necessarily with dr most people after all these years know how to diet with out dr. who knew that you would ever have to prove this to anyone i have been on every diet out there. but once you are diabetic you follow the diet sheet the dr gives you. b.b. my email is bbrown48030@comcast,net if you prefer to do that thanks so much everyone    — parksurf (posted on February 23, 2006)

I'm not sure they're looking for doctor supervised dieting. I was required to provide approximate dates for times I belonged to weight watcher, tops, nutrisystem, etc. I hope this helps.

Check with your insurance company - I know with mine I had to give a list of diets, when I was on them, what I lost to my PCP and then I had to do 6 months in a row of weigh in's with my PCP and that was with Aetna insurance. But, different insurances and different policy's require different things.
